DOC Season 3 Episode 4 Review: Amy Questions Ben as Sonya and TJ Face Parenthood

Things are getting complicated at Westside Hospital and I'm not just talking about the medical cases. Between Amy's increasingly complicated history with Ben, Jake finding himself in hot water and two very different journeys toward parenthood, DOC Season 3, Episode 4, “Great Expectations,” gives us plenty of relationship drama to unpack.
This week's episode puts some of its biggest relationships under a microscope and what's interesting is how much of the conflict comes down to timing, trust and the decisions these characters have made. Whether it's a past Amy can't remember or a future that suddenly looks very different, nobody seems to be having an easy time figuring out what comes next.
Amy and Ben's Past Is Becoming a Problem for the Present
Amy and Ben spend much of the first half of the episode going back and forth over how to treat Bonnie, a patient with an inoperable tumor. Making things even more complicated is Bonnie's connection to Amy's past. She's the ex-wife of Cece, one of Amy's former patients, who has no idea Amy is no longer chief or that she's lost years of her memory.
While Amy and Ben disagree over the best course of treatment, we're treated to flashbacks of their time together at medical conferences. These scenes give us a better understanding of how their relationship developed and just how close they were becoming before Amy's accident.
There's chemistry between them, and the flashbacks make that difficult to ignore. But what's even more interesting is the professional history we're beginning to uncover. Amy was helping Ben with a clinical trial, but she wasn't happy with the numbers. Now Ben is bringing that same trial to Westside, which raises some serious questions about what might happen when Amy starts remembering more of their past.
I have a feeling this clinical trial is going to become a major source of conflict between them. Could Ben end up manipulating the numbers to get the results he wants? That's certainly where my mind is going, especially knowing Amy already had concerns about the data. And what happens if Amy remembers something that could jeopardize everything Ben is trying to accomplish?
That's what makes their dynamic so intriguing. There's this undeniable personal connection, but professionally, these two could be headed for a major showdown. Amy might not remember everything about their relationship, but the more she learns, the more complicated things are going to become. I'm interested to see whether their chemistry survives whatever secrets are waiting to come out.

Jake's Cowboy Behavior Finally Catches Up With Him
Meanwhile, Jake is dealing with a returning heart patient who nearly died because of a surgical error. Jake isn't about to overlook what happened, and his determination to hold the responsible surgeon accountable ultimately results in the doctor being fired. But while Jake might believe he's doing the right thing, his attending physician isn't impressed with how he handles the situation.
In fact, Jake gets called out for behaving like a cowboy and for seemingly getting away with that behavior because of his romantic relationship with Amy. Whew, Jake is feeling the heat! What's interesting about this situation is that Jake's instincts aren't necessarily the problem. A patient nearly lost their life because of another doctor's mistake, and someone needed to address it. But there's a difference between fighting for your patients and deciding that the rules don't apply to you.
The accusation about Amy also adds another complication to Jake's professional life. Whether he believes he's receiving special treatment or not, the perception is clearly there, and that's something he can't simply ignore.
Jake has already been dealing with plenty of personal drama, and now his reputation at Westside is becoming another problem. It will be interesting to see whether he changes his approach or continues doing things his own way, consequences and all.
Gina's Fertility Journey Takes an Unexpected Turn
Elsewhere, Gina and her wife receive some significant news at the fertility clinic. After Gina recently opened up about her changing feelings toward motherhood, the couple learns that Gina has viable eggs and may be able to carry a pregnancy, while her wife does not have the same option. Unfortunately, the news doesn't exactly bring them closer together.
Gina's wife is understandably frustrated, and some of that frustration comes spilling out as she confronts Gina about making her wait so long to start a family. This is such an interesting storyline because we've watched Gina wrestle with the idea of becoming a mother. For years, she believed that wasn't the life she wanted, and now that her feelings are changing, she's discovering that wanting something doesn't necessarily mean the timing will work out the way she imagined.
There's also the emotional weight of what her wife has been carrying. While Gina was trying to figure out what she wanted, her wife was waiting, hoping they might eventually be on the same page. Neither woman can change the time that's passed, and now they're facing another difficult decision about how to move forward.
I'm curious to see how DOC continues exploring this relationship because it's not simply about whether Gina wants a baby anymore. It's about two people trying to navigate a major life decision after spending years wanting different things.
Sonya and TJ Are Having a Baby, and I'm Already Invested
While Gina and her wife are trying to figure out how to start a family, Sonya is dealing with the reality that she's already pregnant.
And let me tell you, Sonya and TJ are quickly becoming my favorite relationship on this show. These two have only been together for four months, and they were just getting to the point where saying “I love you” felt like the natural next step. Now they're about to become parents, and that's a pretty major development for a relationship that's still relatively new.
What I appreciated about Sonya's storyline this week was her vulnerability. During a conversation with Amy, she admits she's worried about whether TJ will stay with her because he genuinely loves her or because he feels obligated to do the right thing. That's a very real concern, especially when you consider how quickly their lives are changing.
They haven't even had the opportunity to fully figure out what their relationship looks like, and now they're going to be connected through this child regardless of what happens romantically. Of course, when Sonya finally tells TJ she's pregnant, he says all the right things. And I wouldn't expect anything less from him.
But saying the right things and navigating the reality of becoming parents are two very different experiences. There's a lot these two still need to learn about each other, and I imagine this pregnancy is going to bring out sides of their relationship we haven't seen yet.
What makes their story particularly interesting is how it contrasts with Gina's. One couple has been trying to decide when and how to have a child, while another is suddenly facing parenthood much sooner than expected. Both situations come with uncertainty, and both are going to test these relationships in different ways. I'm rooting for Sonya and TJ, though. Their chemistry works, I enjoy watching them together, and I cannot wait to see what happens next.
Final Thoughts
“Great Expectations” gives us several compelling storylines, but what stood out most to me was how the episode explores the different stages of relationships and the consequences of choices made in the past.
Amy and Ben have unfinished business that could become both personal and professional, especially as the clinical trial moves forward. Jake is learning that his willingness to challenge authority might come with consequences of its own, while Gina and her wife are discovering just how complicated the road to parenthood can be.
And then there's Sonya and TJ, who are about to navigate a completely different kind of challenge. I'm especially invested in seeing how their relationship develops because there's something refreshing about watching two characters who genuinely seem to care about each other try to figure things out together.
As for Amy and Ben, I don't trust that clinical trial situation one bit. The flashbacks are giving us more pieces of their history, but I have a feeling the most important revelations are still ahead. Between Amy's returning memories and the changing dynamics at Westside, DOC has plenty of drama brewing, and I'm looking forward to seeing how it all unfolds.
