DOC Season 3 Episode 3 Ends With a Major Sonya and TJ Twist

DOC Season 3 continues making Amy Larsen’s recovered memories everybody’s problem, but “I Know What I Want” turns the tables a little. This time, Amy isn’t simply remembering what she lost. She is starting to figure out what she wants now, which would be wonderful timing if Jake Heller hadn’t just given her a very painful reason to question whether that future still includes him.


While Amy and Jake find themselves treating a woman whose later-in-life sexual awakening forces her to make a major decision about her marriage, Gina confronts her own long-buried feelings about motherhood. Elsewhere, Sonya and Michael try to help a young man whose addiction has exhausted nearly every option his family can think of, while TJ and Ben have a conversation about the sacrifices required to become a surgeon. That last conversation becomes much more interesting once we get to the episode’s final moments.



Amy and Jake’s Patient Knows Exactly What She Wants

Amy and Jake are paired on one of the more unexpected cases Westside has seen lately. Their patient is a woman in her 70s who spent most of her life with one sexual partner, her husband. Now living in a retirement community, however, the couple has entered into an open relationship, and she has been enthusiastically embracing this new chapter.


Her newfound sexual freedom has come with a medical complication after she contracts an STI and learns she will need surgery. Her husband has a much easier solution in mind: stop having sex. Considering they were together exclusively for most of their lives, he clearly sees abstinence as the reasonable answer, but his wife sees things very differently.


For perhaps the first time, she is discovering an entirely different side of herself. She has recently experienced her first orgasm, she is enjoying sex, and she has no desire to close that door simply because her husband is uncomfortable with where their relationship has taken them. Given the choice between giving up her sex life and undergoing surgery, she chooses the surgery, and eventually she makes an even bigger choice by deciding she wants to end the relationship.


It could have easily been played simply for laughs, and the circumstances certainly give the episode some humor, but the story becomes surprisingly meaningful. This woman spent decades living one version of her life, only to discover later that there was something else she wanted. Now that she has found it, she refuses to surrender it simply to preserve the relationship she has always known, and that message lands particularly hard considering the two doctors treating her.

Amy Was Ready to Choose Jake Again

Amy spends much of the episode remembering Jake and their relationship, including his assurances that he would never go back to Rachel. Those memories become increasingly important because Amy has been reaching a realization of her own: she thinks she made a mistake.


After everything that has happened between them, Amy is seriously considering telling Jake that she wants him back. Her feelings are no longer simply fragments of a relationship she cannot remember. Amy is making a decision in the present about the man standing in front of her now, and for the first time it feels like she may genuinely be ready to act on those feelings.


Unfortunately, Jake has something to tell her first. He finally comes clean about Rachel, and for Amy, the timing could not possibly be worse. She was getting ready to open herself back up to Jake, only to learn that the promise she remembers him making is no longer true.


It isn’t simply jealousy that hurts her. Amy had allowed herself to believe there might still be a path back to them, and Jake’s confession arrives just as she was ready to start walking down it. The patient Amy and Jake are treating only makes that realization sting more because here is a woman in her 70s confidently choosing the life she wants after spending decades living differently, while Amy is discovering that the future she had finally decided she wanted may have already changed.



Gina’s Past Comes Back as She Considers Motherhood

Gina’s story also fits perfectly into the episode’s larger question about whether the decisions we make earlier in life have to define us forever. Through flashbacks, we see a younger Gina with Amy during a pivotal period in Gina’s life, as Gina comes out to Amy and makes it clear that she does not want the traditional life that had been expected of her.


Gina says she doesn’t want to marry a man, and she doesn’t want children. For younger Gina, those things feel certain, but present-day Gina isn’t so sure anymore. She is struggling with the realization that she wants to adopt, forcing her to reconcile the woman she is today with the woman who once confidently said motherhood was not part of her future.


The episode doesn’t treat that change as Gina betraying her younger self. Instead, it recognizes something far more human: sometimes what we want changes, and that doesn’t mean the earlier version of us was lying or confused. It simply means life has a way of reshaping us in ways we could never predict.


By the end of the episode, Gina finally tells her wife that she wants to adopt. We don’t get her wife’s answer before the episode ends, leaving Gina’s future hanging in the balance, but telling her the truth is an important step. Gina spent most of the episode wrestling internally with the possibility that she could want something now that she once rejected completely, and now the decision belongs to both of them.

Sonya and Michael Face a Family Running Out of Options

Sonya and Michael are thrown into another painful kind of family crisis when they treat a 28-year-old man struggling with addiction. His family has tried nearly everything, including treatment centers, but nothing has worked, and the situation has deteriorated to the point where he broke into his own family’s home looking for money.


The latest relapse is particularly alarming because his substance use involves something seemingly ordinary: energy drinks. He has discovered that consuming them in a particular way and amount can produce the effect he is chasing, showing just how relentless addiction can become when someone is determined to find another route toward getting high.


Michael and Sonya are dealing with more than a medical problem. They are facing a family that desperately wants to help someone they love but has reached the point where love alone cannot fix what is happening. Everyone involved is exhausted, including the patient himself, and the episode does not pretend there is an easy solution waiting around the corner.


After talking with Gina, the young man eventually comes to an important realization. He needs to take accountability for what he has done, which leads him to turn himself in to the police for breaking into his family’s home. It isn’t presented as some magical solution to his addiction, but it does become his first real attempt to stop allowing everyone around him to absorb the consequences of his actions.


TJ Thinks Sonya Will Understand His Surgical Ambitions

Meanwhile, TJ continues working alongside Ben as he considers what becoming a surgeon would actually require. Ben doesn’t sugarcoat it, warning TJ that surgery means brutal hours, enormous demands, and sacrifices that can easily spill into someone’s personal life.


TJ isn’t particularly worried about that part because, in his mind, Sonya will understand. After all, she is a doctor too, so if anyone knows what an all-consuming medical career looks like, it is another physician. On paper, TJ has a point, but DOC clearly did not include this conversation by accident.


By the end of the episode, Sonya may suddenly have something much bigger to consider than TJ’s working hours. After spending the hour helping someone else through a life-changing crisis, she finds herself staring at one of her own when she takes a pregnancy test and the result appears to be positive.


That ending immediately changes the context of TJ and Ben’s earlier conversation. TJ is thinking about the long hours and commitment required to build his future as a surgeon, confident that Sonya will understand because she shares his world, while Sonya may now be looking at an entirely different version of their future.


The episode cuts before we get much more than the apparent result, leaving plenty of questions heading into next week. Does Sonya want to be pregnant? How will TJ react? What happens to his surgical ambitions if they are suddenly discussing parenthood? Considering these two already have plenty to navigate professionally, this could complicate their relationship in a major way.



“I Know What I Want” Is Really About Being Allowed to Change Your Mind

The smartest thing about this episode is how nearly every storyline circles the same idea without feeling identical. Amy’s patient discovers later in life that she wants freedom and sexual fulfillment rather than the marriage she has always known, while Gina once believed motherhood was not for her and now wants to adopt.


The young man battling addiction finally decides he wants accountability rather than another escape, and Amy realizes she wants Jake again just before learning that Jake has made choices of his own. Then Sonya’s pregnancy test arrives and potentially blows up whatever she and TJ thought their immediate future looked like.


“I Know What I Want” isn’t really arguing that everyone should always know exactly what they want. It is arguing that people change, circumstances change, and sometimes the hardest thing is admitting that something you once believed with absolute certainty is no longer true.


Amy finally knew what she wanted, but Jake’s confession may have come just in time to change her mind again. And if that pregnancy test is really positive, Amy won’t be the only doctor at Westside reconsidering what comes next.
