We’ve been tracking the slow-burn transition of Dr. Joan Ridley for weeks, but as the finish line for her tenure approaches, the fallout is hitting everyone from the boardroom to the floor.
In an episode that traded high-octane medical emergencies for the quieter, more cutting friction of career moves and relationship ruptures, we finally saw the roadmap for Westside’s future. And honestly? It’s looking a little bumpy.
The Interview From Hell (and Michael’s Big Reveal)
Joan Ridley isn't going out with a sunset stroll and a gold watch. Instead, she’s spent her final days calling the staff into her office for what feels like a mix of a deposition and a performance review. While she’s busy grilling Sonya, who, let’s be real, could use a little heat, the real drama is happening behind the scenes with Michael and the Board.
Everyone, including Sonya (who spent a good portion of the hour whining to TJ about it), assumed Amy Larsen was the heir apparent. But Joan and Michael have been conducting interviews, and in a twist that surprised absolutely no one but the characters themselves, Michael hated every single candidate.
Finally, the truth comes out: Michael wants the job. While Joan initially needles him, suggesting he just wants to stay close to Amy, Michael shuts that down with a dose of reality. He loves Amy, sure, but they aren't getting back together "anytime soon." He lays out a legitimate case for why he’s the best fit, and Joan agrees, offering her full endorsement. It’s a major pivot for the "suit" of the hospital, especially when paired with this week’s flashbacks.
A Glimpse into the "Doctor" Michael
One of the highlights of “Next” was seeing the man Michael was before the tie and the spreadsheets. We see him approaching Amy (back in her "Cold Amy" era) with the dream of starting a pediatric cardiology unit.
It was a stark reminder of their shared history and the tragedy that defined them. Amy, being her usual empathetic self, note the sarcasm, told him he only wanted to go into administration because he was doubting his skills as a doctor following the loss of their son. It was a brutal look back, but it framed his current ascent or descent (its a pay cutt) to take over for Joan, something more than just a power move; it’s a reclamation.

The Great Medication Mismatch
Outside the boardroom, the medical cases served as a mirror for the fracturing relationship between Amy and Jake. The "honeymoon phase" didn't just end; it hit a brick wall at sixty miles per hour.
Jake is already reeling from hearing through the grapevine that Amy and Michael played mountain rescue in the woods. Their case of the week involves Hayley, a pregnant teenager caught between divorced parents who are the definition of "polar opposites."
When the pregnancy is revealed to be non-viable, the rift between Amy and Jake turns clinical and personal. Jake advocates for medication; Amy insists on surgery. Amy wants to involve the mother; Jake wants to respect Hayley’s wish to only tell her "reasonable" father.
Naturally, because this is Doc, things go south. Hayley has a bad reaction to the meds, proving Amy was right about the surgery from the jump. While the new meds eventually stabilize her, the damage to Amy and Jake’s professional trust is done. Jake calls her out for playing by her own rules, and Amy fires back about his own penchant for secrets. By the end of the shift, they agree they need space. How long that "space" lasts is anyone’s guess, but the vibe is currently "refrigerator-level cold."
Superhero TJ and the Adoption Fair
In much more heartwarming news, TJ has officially secured the surgical residency he’s been chasing. But before he could even celebrate, he was thrown into a case with Walter, a young boy from a group home with severe arthritis.
Enter "Superhero TJ." After Walter sat in the waiting room for four hours, TJ stepped in, calling Gina for backup. The subplot took a poignant turn when Gina discovered that the "science fair" Walter was desperate to attend was actually an adoption fair. Seeing Gina bond with him over space and rockets was the emotional anchor of the episode. Are we looking at a potential adoption arc for Gina? We can only hope, because her "science and space" bond with Walter was the purest thing in the episode.
The Co-Chief Curveball
The episode concluded with Joan delivering a much-needed reality check to Amy. When Amy hears about Michael taking the top spot, she smugly suggests it’ll be nice having “two chiefs.” Joan wasn't having it.
In a moment of peak Joan Ridley, she tells Amy the hard truth: Amy doesn't want to be an intern because she thinks she’s superior. Joan’s final lesson for her protégé? It’s time to put your own needs aside. To ensure Amy actually learns how to lead (and collaborate), Joan names Amy and Sonya as Co-Chief Residents. The look on Amy’s face was worth the price of admission but in the end she seems to understand Joan's point.
Final Thoughts
While "Next" was a slower-paced episode compared to the high-stakes drama of the previous weeks, it did the heavy lifting required to set up the season’s endgame. The power dynamics at Westside have been completely rewritten, and the Amy/Jake/Michael triangle is more lopsided than ever.
As we look ahead, the questions are piling up. Will Amy actually play nice with Sonya? Is Michael ready for the target that comes with the Chief’s office? And what exactly is Nurse Liz planning for Richard Miller?
What did you think of Joan’s choices? Was Michael the right pick to replace Joan, Sould Amy have gotten the job solo? Sound off in the comments and stay tuned to The TV Cave for more Doc updates.
Next Up: How will Amy handle sharing power with Sonya? We'll find out next episode.
